  Checkbox

 

  1. This control allows for the addition of a checkbox to your form.  Once a form has been added to an article, either in a row of a table, or in the article where you wish to place the form element, clicking this control present the CheckBox Properties dialog box.
  2. Enter a Name:   this will be used to identify that field in the email which will be sent.
  3. Choose if Selected is the default state:
      1. Checked—the form will show this checkbox as pre-selected.
      2. UnChecked—the form will show this checkbox as not selected.
  4. Click OK to save.
  5. Click Cancel to cancel the entries.
